This position is an exciting LPN or RN Health Coach opportunity to work with individuals of all ages who are in need of behavioral health care. Must be able to work independently and as a team, be organized and enjoy providing exceptional customer service!

This position will primary support our Psychiatry clinic, with possible coverage in other behavioral health programs. This position is posted as a full time position, we are offering interviews to applicants interested in working 4 days per week on up.

The RN Health Coach utilizes a population health perspective to guide care across the continuum and support patient care transitions.

Job Description

Serves as a leader by demonstrating independence, assertiveness, critical thinking, problem solving skills, and expertise in ambulatory quality measures. Collaborates with the healthcare team to implement improvement strategies that guide optimal care to patients. Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. Collaborates with patients, families and the care team using education, goal setting and motivational interviewing to promote a healthy lifestyle. Must be organized, self-motivated, self-directed, and autonomous; willing to teach the healthcare team; and seek professional development in areas of expertise. Works with the clinical team, oversees the disease registries and promotes the use of Sanford approved Clinical Practice Guidelines and Protocol Orders.

Qualifications

Health Program Coordinator

Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

Three years of clinic/hospital setting preferred.
Currently holds an unencumbered Licensed Practical Nurse (LPN) or Registered Nurse (RN) license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Basic Life Support (BLS) for health care providers required or obtained within the first 60 days of hire. Obtains and subsequently maintains required department specific competencies and certifications.

RN Health Coach

Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing preferred.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
