Job Summary

\n

As a Transplant Coordinator RN, utilize a nursing model for quality patient care. Assess patient needs and create personalized care plans. You'll watch how patients respond to personalized care. Ensure patients have smooth transitions through healthcare. Prevent complications or delays by understanding needs.

\n

What You'll Do

\n\nAssessing transplant eligibility for patients involves interviewing them and their families.\nYou will coordinate tests for transplant candidates, identify any issues in results, and arrange necessary care. You'll work closely with patients, physicians, and other healthcare providers during this time.\nManage recipient's post-transplant care.\nArrange admissions, lab tests, clinic visits.\nCoordinate with referring, primary care. - Understand test results, identify irregularities.\nSet up needed care as directed.\nTeaching candidates, families, caregivers about evaluation, organ allocation, waiting, post-operative, immunosuppression effects, complications, and psychological impact of transplant.\nUsing insurance knowledge to help with post-transplant prescriptions. Assisting patients with coverage issues.\nMaintaining long-term prescriptions.\nComplying with various regulatory policies and maintaining records on the transplant waitlist through different online programs.\nCoordinating recipient transplants includes using the donor website and communicating with potential recipients. Assessing their health status and notifying hospital departments are important tasks. Updates are also sent to the team.\n\n

What Will Make You Successful In The Role

\n\nComprehensive skills in nursing and patient care standards and procedures.\nKnowing rules, laws, regulations, standards, guidelines, certifying bodies, accrediting bodies relevant to our department is important.\nKnowledge of medical terms, health practices, disease control, medication specs, and side effects is essential for healthcare professionals.\nRespect for, and diligent observance of, professional nursing ethics and patient privacy rights.\nExcellent communication skills, both spoken and written, for clear transmission of ideas.\nBasic computer skills: Microsoft Office, information security, scheduling systems, payroll systems, electronic medical records, and email use.\n\n

Qualifications

\n\nAssociate's degree level.\n(2) Two years of professional experience\nA RN License.\nBasic Life Support skills required or obtain BLS certification within 30 days if not already held.\n
