Position Overview We are seeking an independent, adaptable RN who thrives in a collaborative environment, champions teamwork, and brings resilience and professionalism to every interaction. If you’re passionate about patient advocacy, critical thinking, and creative problem-solving, this role is for you! Responsibilities: + Provide direct, compassionate patient care in home settings using the nursing process. + Collaborate with patients to identify barriers and create personalized care plans. + Prevent hospital readmissions through proactive care and education. + Complete OASIS assessments and medication reconciliations in an electronic medical record system. Qualifications + Required: + Current Minnesota RN license. + Ability to obtain American Heart Association BLS certification within 30 days of hire. + Valid Minnesota driver’s license. + Preferred: + Previous experience in Home Care and/or Hospice settings. Key Skills: + Compassionate and patient-focused care. + Strong communication and interpersonal abilities. + Confidence in teaching and guiding patients on disease processes and care plans. + A commitment to quality and excellence. The pay range for this role is $33.66 to $49.65.
