RN Home Health - PACE/ElderPlace (Laurelhurst area)

Job Description:

The Community Care RN provides and supervises professional, independent nursing care for Providence patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence.

Required qualifications:

Upon hire: Oregon Registered Nurse License  Within 30 days of hire: National Provider BLS - American Heart Association  2 years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Preferred qualifications:

Bachelor's Degree in Nursing. Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
