We are currently seeking a dedicated and experienced Home Care RN to join our team and make a meaningful difference in the lives of those we serve.
Job Description:
Position Summary: As a Home Care Registered Nurse (RN), you will be responsible for delivering comprehensive nursing care to patients in their homes. You will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans. Your role will involve assessing patient needs, providing direct care, and educating patients and their families on health management.
Key Responsibilities:
+ Conduct thorough assessments of patients' health status and develop personalized care plans.
+ Administer medications, treatments, and procedures as prescribed by physicians.
+ Monitor and document patients' progress and adjust care plans as needed.
+ Provide education and support to patients and their families on managing health conditions and promoting wellness.
+ Collaborate with other healthcare professionals to ensure coordinated and effective care.
+ Maintain accurate and up-to-date patient records.
+ Ensure compliance with all healthcare regulations and standards.
Qualifications:
+ Current and valid RN license in California.
+ Minimum of 1 year of nursing experience in home care or a related field.
+ Strong clinical skills and the ability to work independently.
+ Excellent communication and interpersonal skills.
+ Compassionate and patient-centered approach to care.
+ Ability to travel to patients' homes within the designated service area.
+ CPR certification
