Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. Cleveland Clinic currently serves 13 counties in the greater Cleveland and Akron areas. Divided into four teams (East1, East2, West and South), each team is then further divided into smaller service areas.
As RN Homecare Float on the South team, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. You will provide assessments, complex nursing treatments and education to help avoid readmissions to the acute care setting. After seeing patients, you will complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules remotely. In the absence of visits, RNs monitor quality through a variety of means, such as chart audits and supervisory visits.
A caregiver in this position works weekends from 10:00AM - 10:30PM.
A caregiver who excels in this role will:
Provide direct nursing care in accordance with established policies, procedures and protocols of the agency, taking visits on the uncovered list, late admissions and other patient visits as needed. Participate in and support quality improvement projects. Participate in departments and committees as assigned. Supervise starts of care, resumptions of care and routine visits as requested. Perform accurate and complete admission assessments and reassessments within a specified time frame. Assess patient for safety risks, abuse and neglect. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess and document patients' pain using appropriate pain scale and manage patients' pain then. Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Escalate change in patient condition to patient’s physician as appropriate.Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of Professional NursingCurrent state licensure as a Registered Nurse (RN)One year of current clinical experience as an RNOne year of experience in an acute medical/surgical or homecare settingValid Ohio Driver’s License and proof of automobile insurance coverage with $100,000/$300,000 coveragePreferred qualifications for the ideal future caregiver include:
