ResponsibilitiesAssesses and evaluates the needs of patients with life threatening and life limiting illnesses and their families and develops an appropriate plan of careIdentifies teaching needs and plans appropriate strategies for individuals and familiesMonitors quality and appropriateness of services providedCollaborates with patient and family, other members of the disciplinary team and other health care providers in providing patient and family careDocuments all changes in patient’s condition and updates care plans and outcomesAdheres to agency infection control and safety policiesAdheres to the organization’s policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position Qualifications NJ RN license (BSN preferred) Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred1+ year of supervised clinical experience as a registered nurseExcellent communication skillsStrong Computer skillsExcellent organizational and interpersonal skills
Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
