RN Hospice Coordinator

Through our Center for Connected Care, we provide a full continuum of care so that patients can recover from illnesses or injuries in the very best location for their individual needs.
A caregiver in this position works, 8:00am—8:30pm, variable days. Orientation will be 3 days a week for 12 weeks.
A caregiver who excels in this role will:
Act as a member of the interdisciplinary team for the assigned functions related to the coordination of services with internal and external referral sources.
Be responsible for processing inpatient referrals into hospice, ensuring continuity of care, and coordinating discharge planning in contract facilities.
Actively engage and educate potential customers to the benefits and appropriateness of hospice care.
Ensure the patient and their family have adequate knowledge and an understanding of the hospice benefit.
Process inquiries for all new referrals (internal and external).
Ensure a seamless transition from hospital to the community setting by coordinating the following hospice services at time of discharge: ordering medical supplies and DME, hand-off communication with the field team and documentation in the electronic medical record.
Perform assessment of potential patients consistent with Cleveland Clinic Hospice policies, procedures and admission criteria in conjunction with the patient’s physician.
Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame.
Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management.
Perform daily assessment and coordinate the plan of care with inpatient staff for General Inpatients (GIP).
Assess patient for safety risks, abuse and neglect.
Assess and document patient’s pain using the appropriate pain scale and manage patient’s pain.
Document changes in patient condition and effectiveness and timeliness of care/interventions.
Evaluate, update, and revise the plan of care to facilitate achievement of expected outcomes.
Know action, usual dosage, indications and side effects of medications administered.
Assume responsibility for coordination of care for managing a caseload of patients in the inpatient setting.
Be flexible in setting and revising priorities for one’s daily assignment based on patient needs/acuity, family requests and available resources.
Attend interdisciplinary meetings as assigned.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of professional nursing.
Current state licensure as a Registered Nurse (RN).
One year of current clinic experience as an RN.
One year of experience in an acute med/surg or critical care setting.
Basic Life Support (BLS) certification through the American Heart Association (AHA).
Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Ability to build relationships with patients, physicians, community agencies and institutions.
Knowledge of JCAHO and Medicare standards and regulations.
Ability to work irregular/varying hours as needed.
Ability to travel to hospitals and community referral sources.
Preferred qualifications for the ideal future caregiver include:
Bachelor of science in nursing (BSN).

Physical Requirements:
Requires full range of body motion including handling and lifting packages, manual and finger dexterity and eye hand coordination. Occasionally lifts and carries items up to 50 pounds. Requires corrected vision and hearing to normal range.Requires standing and walking for extensive periods of time.
