Fulltime Monday- Friday 8-5

Nurses, Home Health Aides, Case workers, Chaplains and Volunteers all work together to provide professional compassionate care.

\n

RNs have autonomy, no office- start and end at home, limited call, no weekends (except weekend positions)

\n

12-16 patients per RN with anticipated 2-4 visits per day.

\n

Shared rotating secondary call is required

Job Summary

\n

As a Hospice Registered Nurse, one coordinates patient care with a patient-centered approach and evaluates health responses. They craft care plans and track patient progress.

\n

What You'll Do

\n\nDevelop, implement, and revise care plan for patient outcomes.\nYou will evaluate the quality and effectiveness of nursing practice, including delicate indicators. You will support evidence-based practice changes through research. Quality/performance improvement initiatives are part of your role.\nYou must create a safe, helpful environment for patients, families, and team members. Build therapeutic relationships, manage pain, and set boundaries to succeed in this role.\nYour role will encompass respecting and incorporating a patient's and family's unique differences into an individualized plan of care.\nMaintain patient and family privacy, autonomy, dignity, and rights. Create a plan reflecting their values and goals, resolving ethical or clinical issues.\nYou will help educate patients, families, and team members to make informed choices about healthcare and treatments.\nYou will work with others to create a care plan, being open and sensitive to everyone's ideas.\n\n

What Will Make You Successful In The Role

\n\nA deep understanding of medical ethics and patient privacy rights is essential.\nEffective communication underpins all aspects of nursing.\nBeing sociable and having the skills to interact with a variety of people will boost your performance in this role.\nObserving changes in patients' medical conditions and effectively relaying this information to other nursing staff and care providers is vital.\nProvide patient-centered care responsibly, adjusting to each patient's needs and experiences with respect and consideration.\nComputer skills are important. Knowing Microsoft Office, scheduling, payroll systems, emr, and email is crucial for the role.\n\n

Qualifications

\n\nRN Associate's degree.\nOne year relevant work experience needed. Highly prefer hospice or home health experience\nMust be a registered nurse. Obtain Basic Life Support certification within 30 days of hire.\n
