Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 


Jordan Health is now seeking a RN Infectious Disease who, under the guidance of the Clinical Services Manager, is primarily responsible for assisting and supporting the PPC practice team by performing targeted task within RN scope as assigned by clinicians and/or practice manager, all with the objective to ensure efficient and quality patient care.

Requirements

  

The RN Infectious Disease will ensure that:

· Clinician burden relative to non-value added task is reduced or eliminated. 

· Jordan patients receive the highest quality care in the most efficient manner when the clinician has all resources necessary. 

· Jordan teams are highly efficient and quality driven. 

· Jordan protects and values compliance with all regulatory and safety requirements at all times. 


Education And Experience Required:

· RN, minimum 2 years’ experience.

· Bachelor’s Degree preferred.

· Excellent computer skills a must, preferably with an electronic health record. 


Licenses And Certifications: 

· Current NYS RN License without any negative actions.

· Current provider BLS/CPR through the American Heart Association. 

· Infection Control Certification. 


Special Skills, Knowledge Required:

· Public health experience.

· Care coordination experience.

· Experience managing patients with HIV/AIDS, Hepatitis C.
