Responsibilities
Job Summary / Purpose
Works with staff at referring facilities to identify and assess potential patients Long Term Acute Care Services; Conducts physical assessments of potential patients to ensure medical necessity for Long Term Acute Care; Acts as the primary liaison between the patient, referring facility, physicians, payers and the Continuing Care Hospital. Prepares, educates and otherwise facilitates transitions for patients from acute care to the Long Term Acute Care setting; Ensures any special needs of the patient are properly arranged for prior to the patients admission; Provides necessary insurance information on potential patients to Insurance Verification and obtains all necessary precerts prior to a patients admission; Gathers clinical and non-clinical patient information to complete a required online Preadmission Assessment for all incoming patients; Conducts and coordinates informational/marketing programs to referral sources as needed to maximize referrals to The Continuing Care Hospital.
Essential Key Job ResponsibilitiesUses strong clinical knowledge, communication skills, and problem-solving abilities to build productive relationships with physicians and discharge planners at hospitals. Be results focused in order to meet census goals.Meet with referring hospitals every day to help identify and transition patients who will benefit from LTACH servicesPerform pre-admission assessments on referred patients.Educate patients, families, hospitals and healthcare professionals on the benefits of LTACH care.Monitor marketing activity through call logs (activity logs) and profiling of customers is a necessary skill. Cultivating relationships with sources to generate repeat referrals.Providing facility tours and conducting presentations for potential referral sources and patient families. These tours may occur after normal business hours depending on schedules and availability of patients’ families. Attending seminars and conferences to promote facility.Communicating activities and concerns to supervisor.Completing required paperwork, managing correspondence and maintaining data. Minimum Qualifications
Required Education and Experience
A minimum of an Associate Degree in Nursing from an accredited nursing program is required. All new-hire Associate Degree Registered Nurses (new graduate and experienced) are required to start an approved BSN program within two years of hire and complete an approved BSN program within five years of hire.
Required Licensure and Certifications
Kentucky RN License, Valid Divers License
Required Minimum Knowledge, Skills, Abilities and Training
Clinical assessment skills for Long Term Acute Care Criteria, sales and marketing skills. Excellent organizational ability. Excellent communication skills with a variety of medical and non-medical personnel. Ability to work independently and manage your workload to achieve goals. Flexibility to adapt to the changing regulatory environment of long term acute care hospitals. Equipment: PC software, word processing, telephone, copier, fax, automobile. Adequate conversational English is required in order that the individual be able to take direction from management, understand how to complete job tasks, communicate adequately with patients and co-workers as necessary, understand and follow safety guidelines, and understand the Associate Handbook and all hospital policies.
