Major Responsibilities:Provides patient and family with education and supports them throughout the diagnosis, treatment and follow-up of the patient. Provides timely, seamless, evidence based care resulting in best patient outcomes.Completes a nursing assessment to identify the patient’s individualized needs and collaborates with the care team to develop and execute a care plan. Monitors progress of patients and collaborates with other team members to ensure continuity of care and smooth transitions between care settings.Assists physicians and Advance Practice Providers (APPs) before/during/after clinic visits or inpatient rounding, and arranges for appropriate testing and consultation. Communicates with all parties such as providers, patients, financial advocates, pharmacists, and insurance companies regarding patient condition and needs. Contacts providers regarding additional testing or medications needed and/or medication adjustments.Follows protocols for monitoring results, identifies clinical issues, and seeks physician/APP assistance as needed. Assists providers in executing individualized treatment plans by assisting with accurate ordering of chemotherapy doses, radiotherapy doses, medication, IRB case submission, procedure scheduling, and ensures appropriate arrangements for treatment or post-procedure care.Evaluates, selects or designs patient educational material on specific disease site programs. Provides expert clinical advice in designing collaborative teaching plans/programs and in planning/providing education and discharge planning; including wellness and health maintenance within designated specialty. Provides education and outreach programs to patients, Aurora staff and to members of the community.Works collaboratively with scheduling staff to ensure that evaluations, patient testing, and treatment are provided in a timely manner and in accordance with established protocols.May be accountable for all clinical documentation required for database management. May prepare or submit cases as required. Monitors, evaluates, and communicates data regarding clinical problems to appropriate members of the care team.Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, or Oncology nurse (OCN) certification issued by the Oncology Nurse Certification Corporation (ONCC) needs to be obtained within 2 years.
Education Required:Associate's Degree in Nursing.
Experience Required:Typically requires 5 years of experience in clinical nursing that includes performing nursing assessments, care plan development and educating patients.
Knowledge, Skills & Abilities Required:Working knowledge of the components of quality and acute patient care needs specifically related to oncology/interventional oncology.Excellent analytical and interpersonal communication skills necessary to effectively work with patients, families, and care teams.Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.Proficient computer skills in Microsoft Office (Word, Excel, PowerPoint, Access) or similar products.Ability to work independently, handle multiple tasks, set appropriate priorities, and accomplish assignments in a thorough and timely manner.
Physical Requirements and Working Conditions:Clear oral communications and hearing acuity required for receiving instructions and converse on standard telephone. Must be able to:lift up to 50 lbs from floor to waist. lift up to 20 lbs over the head.carry up to 40 lbs a reasonable distance.Must be able to:push/pull with 30 lbs of force.perform a sliding transfer of 150 lbs with a second person present. Personal Protective Equipment worn as necessary with patients. May be exposed to blood and body fluids.Operates all equipment necessary to perform the job.
