Reports directly to the Director of Orthopedics, Rehab, Sports Medicine, & Neuro.Coordinates patient centered care across all departments from the surgeon office to PAT to surgery to the nursing unit and post discharge. The coordinator is responsible for coordinating and implementing services and activities related to the orthopedic service line programs. Assists the director and physicians with the oversight, development and implementation of the joint, hip fracture, spine and other orthopedic programs as assigned. Responsible for pre-op education of patients, facilitating patient flow in emergent cases such as hip fracture, follow-up post-surgery, data collection and analysis, and orthopedic process improvements involving the entire interdisciplinary team.Works closely with the Medical Directors, hospitalists, and surgeons to meet program needs and facilitate an Orthopedic Center of Excellence. Responsible for pre-operative patient education for joint patients, patient follow-up post-surgery, data collection, analysis, and reporting. Conducts patient follow-up of total joint arthroplasty patients using post-op follow-up visits, phone and written survey tools. Communicates regularly with the interdisciplinary team to coordinate the delivery of orthopedic care to patients across the continuum.Assists the orthopedic service line director with the Orthopedic Council meetings and acts as a liaison between administration, clinical departments and the orthopedic surgeons.Qualifications
