EXPECTATIONS
-RN will be a 1.0 FTE
-RN may occasionally be assigned a call shift
-RN may occasionally be asked to assist on a weekend (extenuating circumstances only)
-RN will be a part of the holiday rotation
-This position will work closely with their direct team, the other leads, and the supervisors.

-function as triage at branch level (currently downline)

-assist in medlcine orders
-ensure new admissions have necessary orders (meds, DME, visit sets, etc...)
-assist in coverage of other leads in their absence

Job Summary:
Provides leadership and coordination of clinician/patient staffing and/or scheduling while ensuring implementation of clinical objectives and quality patient care to patients within a home setting or an SSM hospice house location.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
Assigns, leads, educates and monitors nursing and support staff during assigned shift. Contributes to or performs performance evaluations of staff.Functions in the role of supervisor or branch manager in branch leader absence and takes a lead role in process improvement responsibilities. Assists branch leaders in preparing clinical reports and operational duties.Assesses daily patient care needs and develops and distributes patient care assignments, ensuring that population appropriate patient care is given. Performs direct patient care as needed.Coaches staff on effective and efficient case management & collaborates with other health care providers to deliver results for positive clinical & financial outcomes.Collaborates with advanced practice providers to ensure quality outcomes.Evaluates the effectiveness of patient care and identifies any issues that affect desired patient outcomes.Responds to complaints about patient care and manages through established channels.Performs ongoing chart review on case load with attention to: a) Accurateness of 485, recertification, verbal orders for changes in Plan of Care. b) Assuring every visit performed is within assigned frequency. c) Assuring overall documentation supports coverage of service as defined by regulation or payor.May provide input for hiring, training, directing, developing and evaluating staff.Works in a constant state of alertness and safe manner.Performs other duties as assigned.
