The data collected forms the basis of an individualized plan of care. Identifies teaching needs and goals related to the diagnosis of each patient from the nursing admission assessment and by\ncollaborating with patient/family and the Interdisciplinary team.\nProvides patient information and status in a timely manner for the Initial Team Conference and for Re-evaluation Team Conferences on a weekly basis. Responsible and accountable for accurate succinct and timely documentation of all assessments interventions and patient’s response in the medical record in accordance with policy.\nParticipates in the patient rehabilitation program on an assigned unit. Is responsible and accountable for direct patient care by utilizing the nursing process of assessing planning implementing and evaluating patient care with a priority on the education of the patient/family/significant other based on the health care needs identified. Uses a positive and encouraging approach to achieve patient goals and quality outcomes.\nInitiates discharge planning on all patients from the day of admission. Initiates patient teaching in anticipation of the discharge date.\nCommunicates all pertinent information regarding changes in patient’s status from one care provider to another to support continuity of care and patient safety. This includes the attending physician and the Charge Nurse. Seeks consultation from more senior nursing staff in support of complex clinical decision-making.\nProvides direction training and supervision of ancillary nursing personnel by utilizing problem solving techniques in decision making organizing setting priorities and using time effectively. Delegates appropriately.\nProvides for and assures personal and environmental safety in accordance with hospital and nursing policies and procedures.\nPositively contributes to the functioning of the unit by providing calm direction and reassurance to ancillary staff other departments\npatients and families. Participates in team building activities on the unit to create a positive and efficient work environment.\nMaintains communication with manager and senior nursing staff related to service needs such as necessary supplies equipment including repairs and clinical training in order to effectively complete assigned duties. Provides care in a cost effective manner while promoting quality patient outcomes monitors supplies and charges.\nAssumes charge responsibility following orientation period as requested.\nIdentifies and initiates own professional growth actively pursuing professional development through continuing education and self\nstudy.\nPrioritizes activities and plans for effective use of time and staff in meeting patient needs.\nPerforms other duties as requested. Position Skills: BLS Certification Required Prior to First Shift, Required | ACLS Required Prior to First Shift (CIRH ONLY), Required | Minimum Two (2) Years Bedside Clinical Experience, Highly desired Assignment Details: Shifts/Wk - 3 Hours/Wk - 36 Shift Hours - 12 The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings. Essential Duties and Responsibilities: + Provides planning and delivery of direct and indirectpatient care + Responsible for providing care to medically complex patients + Supervises care, treatment, and services of the patient(s) + Documents the patient’s plan of care using identifiednursing diagnosis, expected patient outcomes and selected nursing interventions + Maintains confidentiality at all times with patients,medical staff and co-workers + Provides a safe, comfortable and therapeutic patientenvironment + Adheres to and supports thepolicies/procedures/goals/objectives of the hospital in an attempt to providequality patient care + Revises plan of care according to evaluation, changes inplan of care and nursing interventions + Performs interventions according to identified priorities,plan of care, and facility policies and patient care outcome standards + Identifies and prioritizes actions based upon patient carerequirements Minimum Requirements: + Current RN license for the state in which the nursepractices in + One (1) year nursing experience in a long-term acute carefacility preferred + Experience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patients + Complies with all relevant professional standards ofpractice + Participation and completion of Amergis' Competency programwhen applicable + Current CPR if applicable + TB questionnaire, PPD or chest x-ray if applicable + Current Health certificate (per contract or stateregulation) + Must meet all federal, state and local requirements + Successful completion of new hire training as applicable tojob site + Understand patient confidentiality and HIPAArequirements + Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required + Computer proficiency required + Must be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: + Competitive pay & weekly paychecks + Health, dental, vision, and life insurance + 401(k) savings plan + Awards and recognition programs *Benefit eligibility is dependent on employment status.
