THE UNIT:

AGH's team of Heart Transplant Coordinators serve as liaisons between patients and the entire transplant team, working to coordinate, evaluate and prepare the patient for heart transplant. In addition, the Heart Transplant Coordinators play an integral role in coordination of post-transplant care.

Job Summary: Responsible and accountable for a designated patient population for the purpose of providing care based upon enactment of the nursing process and floats to multiple areas within an assigned team based on skill level.

GENERAL OVERVIEW

Applies clinical expertise in evidence-based nursing practice. Integrates care with multidisciplinary team (within the hospital and in the community) to facilitate MCS patients care from pre-implant through explant.

ESSENTIAL RESPONSIBILITIES

Ensure all program and regulatory requirements are met for the emergent and pre-planned BTT or DT VAD insertions.

 Facilitate the discharge, education and transition of the MCS patients from the inpatient to the outpatient care.

 Coordinate the outpatient care of the MCS patient.

 Assist with the coordination of staff educational programs related to the Mechanical Circulatory Support Program.

 Ensure communication of MCS patient's needs with local ER's EMS and electric companies.

QUALIFICATIONS

Minimum

BSN or Bachelor's degree

PA registered nurse

5 years of nursing experience

2 years nursing experience in cardiovascular critical care or cath lab. 

Experience in developing policies, procedures and guidelines.

CPR – American Heart Association

Act 34 Criminal Background Clearance Certificate

Act 33 Child Abuse Clearance Certificate

Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
