Full or Part Time RN Needed + 1 FT and a need for a PRN/PT + One for reaching Baltimore County and Baltimore City + 40/50 patients a month + Microsite and micro introductions vs social economic factors + Bedside and In-home manor bringing both things together. + Face to face time is the first 45 initial assessments and wrap up charting about 15 minutes after + Forms are fairly simple and targeted templates on care specifics. + Technology would be provided and training during the onboarding + 2 – 3 (8 hours shifts of training) company and HIPPA training and other items including EMR - Remote + About 4 – 6 visits to max with then charting afterwards + Hi model and Hi touch care where you live for high risk Medicare members with an MCO program (will have their own OB and will be adjunct care and social care coordination) + Align those with programs for additional help as needed. + Make referrals through coordination with program director and how to ensure visits will be made + Mom and baby are both members of the MCO program + Knowledge of developmental milestones for up to 12 months + Work with Community Health Workers and Doulas as well Amergis Healthcare Staffing is seeking a Registered Nurse to assume the responsibility and accountability for the applicationof the nursing processand the delivery of patient care for post-partum and newborn patients. The nursedemonstrates ability to make clinical judgments in an effective and efficientmanner with supervision. The nurse demonstrates critical thinkingand performance ability in the coordination of patient care. Consistently performsaccording to nursingstandards and is accountable in managing patient care and assisting others inthe management ofpatient care. EssentialDuties and Responsibilities: + Providesnursing care to the post-partum and newborn patients. + Utilizesthe nursing process to assess, plan, implement and evaluate patient care. + Performadmitting assessments, review history and develop plan of care. + Collects,analyzes and interprets data and information from health care members anddocuments actual and/orpotential nursing diagnoses. + Completespatient/family education as required. + Performsinterventions according to identified priorities, plan of care, and thehospital policies and patient careoutcome standard. + Providesa safe, comfortable and therapeutic patient environment. + Monitorsthe safety and effectiveness of the environment and equipment. + Promotesinterpersonal relations with patient/family/significant others; hospitalhealthcare co-workers; and physicians. + Useseffective communication skills with all contacts while being objective andsensitive. + Communicatespertinent information about patient, nursing unit, and activities asappropriate. + Represents Amergis in the hospital setting in a positive image. + Afteraccepting a Amergis hospital assignment, arrives at the hospital on time, beforethe scheduled shift startsto sign-in at the nursing office and report to the unit/floor for orientationand/or report. + Otherduties as assigned. MinimumRequirements: + CurrentRegistered Nursing license within the state of practice. + Minimumof one year mother / baby experience preferred. + Minimumof one year acute care experience within the last 3 years preferred. + CurrentBLS Card. + CurrentHealth Certificate (if applicable). + CurrentPPD or chest x-ray. + Fullcommand of the English language, both verbal and written. + Demonstrategood interpersonal communication skills. + Demonstratethe ability to effectively use the nursing process.
