Responsibilities
The Nurse Navigator is assigned to a population of patients and provides individualized and comprehensive care to reduce barriers coordinate diagnostic and treatment efforts and ensure prompt access to care. Applying the nursing process the Nurse Navigator performs clinical assessments; education on disease process plan of care and treatment modalities; and symptom management to support patients through the complexity of their care continuum. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to effectively coordinate patient care ensure continuity of care and create a comfortable and exceptional patient experience. The Nurse Navigator initiates referrals to appropriate inter-disciplinary teams for physical psycho-social and spiritual support through all stages of the continuum of care.
Provides coordination of care and support for patients, families and caregivers throughout the continuum of care.Provides education for patient, caregiver and family on disease process, plan of care, treatment, symptom management and survivorship.Effectively communicates with patient, caregiver, families and multidisciplinary healthcare teams.Provides nursing assessment through a process of gathering pertinent patient's medical history and review of presenting symptoms.Demonstrates professionalism in the workplace with patients, families, healthcare team and community.Provide excellent customer service resulting in patient satisfaction with care.Promotes a patient-and family-centered care environment for ethical decision making and advocacy for patients.Provide a warm welcome to patient and family members with introductions to self and multidisciplinary team members.Communicate time for appointments, procedures and potential delays.Prioritize incoming phone messages and triage according to clinical needs in a timely manner.Return calls within same day of call.Use positive service recovery techniques.Returns patient messages and/or requests within 24 hoursQualifications
MINIMUM
3 years of experience as a RN in an inpatient or outpatient setting.
Completion of accredited RN program and Clinical Experience.
Computer skills EMR documentation
Record/Verify system work experience: patient education/interventions symptom management.
AZ RN license
American Heart Association BLS
