The Perinatal Navigator’s primary function is to guide women and their families through both typical and high-risk pregnancies as well as the postpartum period. The Navigator assists with developing complex care plans and walking patients through the healthcare system by identifying those in need of further counseling or assistance from other medical specialties (lactation, addiction/rehabilitation, social services, developmental). They will also help in developing relationships with service providers and tracking interventions and outcomes from Methodist Women and Infant Services. The Navigator will also develop a resource bank for community services such as financial and insurance needs, and social services programs for any perinatal patients in need.

Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)

*Serves as liaison for inpatient and outpatient perinatal services between the Methodists hospitals, the community, IDPH and other regulatory agencies.
*Acts as the subject matter expert on Indiana Perinatal Standards.
*Works directly with MPG offices to identify and provide ongoing support for high-risk patients pre/postnatally.
Responsible for maintaining compliance with Methodist hospitals perinatal designations.
Provides oversight of quality improvement activities related to pregnant women and newborns.
Identifies and abstracts appropriate cases for quality review.
Collects, analyzes and reports perinatal data to internal and external stakeholders.
Makes follow-up phone calls to patients and physicians to ensure the continuity of appropriate care.

Manage multiple projects independently with an entrepreneurial approach.
Display excellent organizational skills with a focus on details and follow through towards completion.
Develop strong working relationships with volunteers.
Demonstrate strong computer skills, knowledge of Microsoft Office Suite, email, and internet.
Demonstrate clinical competency in patient care, teaching, and management of patients with pregnancy, gynecological, or post-partum issues.
Ability to provide compassionate, kind and a caring approach regarding patient interactions.


EDUCATION

Bachelors Nursing PreferredCollege Diploma Nursing RequiredMinimum of 2 years’ experience in maternal child servicesLicensed Registered NurseCertified Cardiac Pulmonary Resuscitation
