**Job Description:**
Responsible for specialty care management for assigned clinics/geographical areas and other appropriate referrals within the Intermountain system. Assess, plan, educate, and advocate for patients; health needs by collaborating with the patient/family, physician(s), other health care providers, national organizations, and community resources. Collaboratively develop and implement a plan of care that meets the patients healthcare goals, promotes shared-decision making, optimizes allocation of resources, and facilitates patient engagement and self-care management across the care continuum.
**Posting Specifics**
**Shift Details** : Monday - Friday 8:00am - 4:30pm. No weekends or holidays.
**Department** : Outpatient Oncology
**Additional Details** : General Nurse Navigator within the outpatient medical oncology clinic and will navigate patients within the Lung Health program. This includes pre and post patient education and the importance of early lung cancer detection.
This position will require 2 years of oncology experience.
**Scope**
+ Provides guidance to the patient through the care process and plan of care.
+ Assesses patient needs and coordinates education, discharge planning, development of a post-acute care plan, and coordination of post-acute services.
+ Directs follow-up on patients progress relative to their post-acute plan.
+ Questions and prevents duplication, fragmentation, or use of unnecessary resources; consistently questions tests/procedures for which the purpose is unclear.
+ Track and collect data on patients including demographics, length of stay, and reason for extended stay. Participate in readmission analysis on assigned patients with sending hospital and inclusive of post-acute providers. Provides reports as needed.
+ Promotes mission, vision, and values of SCL Health, and abides by service behavior standards.
+ Performs other duties as assigned.
**Minimum Qualifications**
**Required**
+ Graduation from an accredited School of Nursing is required
+ Current license as a Registered Nurse in the state of practice is required Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP)certification upon hire or obtained within 60 days of hire with prior approval (ACLS/NRP/PALS as appropriate for unit or service )
+ Three years RN nursing experience required
**Preferred**
+ Bachelor's degree of Nursing is preferred
+ Certification as appropriate for unit or service is preferred

**Physical Requirements:**
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.- and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.- and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.- and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.- and -Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).- and -May be expected to stand in a stationary position for an extended period of time.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$43.98 - $63.79
