RN-Open Access Triage and Tranisitional Care Management

Sensitivity—Using my senses to perceive the true attitudes and emotions of others.   Summary of Duties and Responsibilities: Responsible for professional nursing care and related telephonic or in person assistance to Variety Care patients/families. This position provides support to staff in Open Access (Urgent Care) and Patient Access Center as well as other licensed and non-licensed staff in the provision of patient care. The Open Access Triage and TCM RN serves as a patient advocate and educator, providing health and wellness teaching while collaborating with other members of the healthcare team to promote successful patient outcomes.   Primary Duties and Responsibilities:  Provides individual patient/family health coaching focused on training and encouragement of the patient into a state of wellness. Reviews available hospital EMR’s and internally generated reports to identify Variety Care patients that have received care as a hospital inpatient (IP) or through an emergency department (ED). Review and import records into VC EMR to inform providers of patient activity. Perform TCM intake on all IP and appropriate ED discharged patients. Create telephone encounters on lower acuity ED patients as appropriate and forward to providers MA. Performs medication reconciliation as part of the TCM intake process. Identifies patient’s personal strengths to educate and recruit participation in improving and proactively maintaining their own health. Teaches and serves as a clinical resource to staff, and delegates in accordance with the competency level of all staff members. Serves as a preceptor/mentor to inexperienced staff and completes the appropriate documentation of orientation, training, and competency. Communicates barriers and patient’s health needs to the appropriate provider when indicated. Interprets reports, data, and medical information to determine the needs of the patient. Provides medication management teaching, as necessary. Accurately documents all patient interactions utilizing the electronic healthcare record (EHR). Complies with all Variety Care policies, including those related to safety, privacy, confidentiality, and security of information. Actively supports Variety Care Mission Statement, patient care standards and customer service standards. Participates in organizational committees and focus groups as assigned. Assists in the training and mentoring of Open Access and Patient Access Center staff. Assists healthcare providers in stabilization of patients experiencing medical emergencies. Participates in clinic wide initiatives and staff training, as assigned. Performs other duties as assigned.   Requirements, Special Skills or Knowledge:  Active Oklahoma Registered Nurse license. BLS-HCP Certification. Strong communication and interpersonal skills. Excellent computer skills.     Preferred Requirements, Special Skills or Knowledge:   BSN preferred. Bilingual (English/Spanish).   Essential Functions: Able to lift a minimum of twenty-five pounds Able to sit/stay at desk for extended periods of time.
