The RN Patient Flow Coordinator at LCMC Health is responsible for the coordination of patient transfers across the health system and strategic bed planning to ensure optimal patient flow and throughput. This role requires critical thinking, problem-solving, and effective communication to facilitate the transfer process and to plan and manage bed capacity across the system. The RN Patient Flow Coordinator will act as a liaison between referring facilities, LCMC hospitals, and clinical staff to ensure timely and appropriate patient placements.
GENERAL DUTIES
Coordinate Patient Transfers:
Serve as the primary contact for all incoming transfer requests and manage communication between referring facilities, physicians, and hospital teams.Use clinical judgment to gather and assess patient data, ensuring appropriate and timely patient transfers across the LCMC Health system.Facilitate safe and efficient transfers by coordinating logistics, including transportation and bed placement, while maintaining accurate documentation.Manage Bed Planning and Throughput:
Monitor and manage bed assignments to optimize patient flow and system capacity across LCMC Health facilities.Timely bed assignments completed to facilitate patient movement.Anticipate future bed needs by reviewing schedules and preparing for upcoming admissions to support seamless patient throughput.Facilitate Communication and Collaboration:
Communicate regularly with key stakeholders to address patient placement needs and capacity concerns, supporting hospital-wide goals for patient movement.Support Capacity Management:
Assist in the proactive management of patient flow by collaborating with house supervisors to address areas approaching capacity.Utilize tools such as the Capacity Dashboards to monitor and manage bed utilization, identifying areas for intervention to maintain operational efficiency.Documentation and Reporting:
Complete intake documentation requirements.Ensure that all activities related to patient transfers and bed planning are thoroughly documented in compliance with LCMC Health standards.Additional Duties:
Engage in ongoing professional development to enhance knowledge and skills relevant to patient flow and transfer coordination.Establishes and maintains a high degree of awareness and knowledge of hospital’s medical staff, services and programs. Maintains professional and technical knowledge through continuous education and accepts new assignments and performs additional duties as requested.EXPERIENCE QUALIFICATIONS
3 years experience in inpatient nursing / call center / critical care / ED/house supervisor
