Responsibilities

Serve as a patient navigator throughout the continuum of care.

Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.Coordinate efforts for treatment and promote quality improvement and educational efforts.Compile data, track outcomes, and make recommendations for process improvement.Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
Requirements

Licensure / Certification / Registration:

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Additional Preferences

Degree in Nursing
Active RN Licensure
3-5 years experience, 1-2 primary care preferred
BLS Certification required

Maintains confidentiality of patient information according to established practice policies and government regulations, such as HIPAAAbility to work independently and exercise clinical judgment in interactions with Physicians, patients and their families.Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components.Aware of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources.Computer Skills – Proficient ability to use a computer and electronic medical record.Teamwork – Work as part of a team and collaborate with co-workers and providers

Essential Function: 

Provides direct patient care

Performs Medicare Annual Wellness visits, coordinates patient care, patient education and communicates with patients, providers and care teams

Proactively acts as patient’s advocate, working to resolve patient concerns

Ensures that documentation and billing of services performed is sufficient to meet CMS guidelines, correct coding guidelines and other payer quality incentive program guidelines

Maintains confidentiality of patient information according to established practice policies and government regulations, such as HIPAA

Performs pre-visit planning to ensure appropriate follow-up and documentation of preventative screeners including health maintenance items

Understands population health management and works to ensure that Medicare patients are managed within ACO program guidelines

Understands preventive care guidelines (e.g. US Preventive Services Task Force, Medicare, etc.) and makes recommendations to patients accordingly

Knowledgeable of community resources and assists patients in connecting with those resources
