RN - Pediatric Palliative Care

Bilingual in Spanish/English strongly preferred.

The Pediatric Palliative Care RN Coordinator has responsibility for coordinating the activities of all palliative care team members and organizing the weekly team conference.  The RN Coordinator is the primary link to referring staff and physicians who are seeking the services of the palliative care team.  The RN Coordinator initiates the response to a request for consultation, assesses the patient, and involves other team members as appropriate and coordinates care through community settings and hospice.  In addition to the clinical duties, the RN Coordinator is responsible for educating staff about palliative care, maintaining data on all patients referred, and participating in quality improvement activities as it relates to palliative care.

Required Qualifications:

Associate's Degree in Nursing. California Registered Nurse License upon hire National Provider BLS - American Heart Association within 30 days of hire 2 years RN experience in the acute care setting or transitional care setting.

Preferred Qualifications:

Bachelor's Degree in Nursing HPNA Certification (eligibility for certification required) upon hire
