You will be enhancing the quality of patient management and satisfaction by promoting continuity of care and cost effectiveness through the integrating functions of case management and encouraging preventative care. Responsible for completing Annual Wellness Visits with eligible patients. Ensures education provided to patient is factual and applicable to the individual. Monitor and manage patients’ Medication Adherence with various payers. Assists patients with identifying and overcoming barriers they may face that prevent adherence to medications.
