With the changing landscape to a value-based service line, attention to quality across the continuum of care will be imperative. The impact of incentive payment on quality outcomes is increasing. This position will help lead the implementation of quality improvement activities through the development, oversight and coordination of detailed work plans and gap analysis of various internal constituents and external agencies. This position assists with the analyses, review trends/research and develops new and more efficient approaches to care of the patient population and other populations as needed to maximize impact on operational efficiency, reduce preventable harm, improve incentive reimbursement and population health.
ESSTENTIAL DUTIES AND RESPONSIBLITIESCoordinate and ensure development and implementation of annual quality improvement plan, evaluation of the annual plan and needed revisions in policy and procedures and practice related to the plan. Develop strategies with teams to ensure patients receive safe and equitable care.Coordinate the preparation of committee agendas and minutes; develop and maintain queue and tracking mechanism to ensure follow up and closure of issues prn.Collaborate with colleagues in identifying quality of care issues, assist in conducting audits and gap analysis resulting in detailed work plans to achieve compliance through team meetings and workgroups. Coordinate with appropriate management and staff to ensure timetables are attainable and met. Coordinate with appropriate departments to assure that all incentive programs are meeting data submission and quality indictor requirements to assure 100% reimbursement of the incentives.Educate or collaborate to ensure colleagues and stakeholders are updated on best practices in caring for key populationsRespond to requests for data from internal and external sources following developed protocols for release of information.Proficient at the use of computer databases, importing and exporting data, report development and basic statistical analysis.Maintain knowledge of current trends in quality improvement and critical care.Interact with Trinity Health Service/Physician Enterprise and multiple external resources related to issues of quality.Maintains the confidentiality of information acquired pertaining to patient, physician, employees and visitors to Saint Joseph Mercy Health System. Discusses patient and hospital information only among appropriate personnel in appropriately private places.Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.Behaves in accordance with the Mission, Vision and Values of Saint Joseph Mercy Health System.Other duties as assigned.REQUIRED SKILLS AND ABILITIES
