Overview RN Case Manager – Full-Time Day Shifts


Location: Jackson, TN

 

Overview:
Adoration Home Health & Hospice is seeking a compassionate and organized Registered Nurse (RN) Case Manager to oversee patient care and provide case management services. This full-time position offers a consistent weekday schedule, allowing you to make a meaningful impact in your patients' lives across multiple counties.

 

Schedule:

Monday through Friday: 8:00 AM – 4:30 PM

Service Areas:
This role will serve patients in Haywood, Crockett, Madison, Lauderdale, Tipton, Henderson, Decatur, Chester, Hardin, McNairy, and Hardeman Counties.

 

Responsibilities:

Develop, implement, and monitor individualized patient care plans.Coordinate care with interdisciplinary teams to ensure the best outcomes for patients.Educate patients and families about care plans and available resources.Conduct regular assessments and provide support for patients in their homes.

Certifications Required:

Active RN license in Tennessee.Current CPR certification. Responsibilities Conducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends and oversees the implementation of improvement plans Collaborates with Branch Director in the implementation of quality improvement activities and corrective action plans Provides feedback to Operations management regarding necessary changes and revisions relating to care provision Accountable to ensure all clinical staff appropriately and successfully complete orientation and all required in-services Responsible for the oversight of and response to external and internal surveys and audits within assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadership Acts as a resource and has a working knowledge of payer requirements relating to the provision of care and documentation guidelines Completes documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and the organizationHolds case conferences to ensure oversight of care, coordination of services and that standards are metEnsures documentation standards are met per company standards and to support quality standards, coverage, and reimbursementEnsures that care is coordinated with all involved in patient careIdentifies variances in standardized care and care processes to identify areas for improved performance related to patient careEnsures compliance with regulatory, payer and company requirementsFacilitates the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews) Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standardsOversees and directs the selection and hiring of clinical staffConducts interviews and makes hiring decisionsEnsures staff competency and performanceEnsures assigned staff are evaluated fairly and timely in accordance with company performance evaluation proceduresEnsures direct, on-site supervision for all staff per company requirementsResponsible for achieving organizational performance and retention goals among clinical staffServes as a resource in assisting the field staff with problem solving to provide the most appropriate care for the patientServes as a coach, mentor, and role model to all professional and direct-care staffEnsures adequate numbers of preceptor staff to provide development and mentoring of staff clinicians and direct-care staffAt least one preceptor clinician is trained to provide back up for the clinical manager position Other duties as assigned – may be asked to perform patient visits and perform on-call/after-hours duties in the event of staffing issues Qualifications Bachelor’s Degree preferred in Nursing or Physical Therapy from an accredited school of nursing or therapyLicensed Registered Nurse (RN) or Physical Therapist (PT) in good standing and currently licensed by the State Less than two years clinical experience, and at least one year in related management/supervisory role in home health care Current CPR certification Must meet all agency requirements for pre- employment as required by the company and/or State regulations Ability to use company documentation systems Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company Ability to work independently as well as part of a teamCapable of working responsibly with highly confidential About our Line of Business Adoration Home Health and Hospice provides quality and compassionate services in the comfort of home, providing support for patients, families and caregivers in their time of need.  Adoration was formed to fill the need for a loving, community-focused, caring organization.  We empower patients to live with dignity, find a sense of fulfillment, and celebrate with their families a life well-lived.  Our employees and caregivers are proud to be a part of the Adoration team and the mission of our company.  For more information about Adoration, please visit www.AdorationHealth.com.  Follow us on Facebook and LinkedIn.
