This position has a $5,000 Sign On Bonus!
This will be paid out in four equal payments, as long as the employee is in good standing and working as a Full Time Resident Assessment Coordinator.
SUMMARY
The Resident Assessment Coordinator, RN supports the applicable clinical software used in assessments and care planning, with a goal of facilitating quality assurance.
PRIMARY RESPONSIBILITIES
1) Oversee and administer the Resident Assessment process according to applicable policies and regulations. *
a. Schedule the ARDs (Assessment Reference Date) for payment assessments in a manner that accurately captures the RUG (Resource Utilization Group) category. *Provide a schedule of ARDs and assessment types to the IDT (interdisciplinary team) to facilitate timely completion of MDS sections and CAAs (Care Area Assessments) by each discipline. *
b. Schedule all OBRA (Omnibus Budget Reconciliation Act) ARDs (assessment reference dates. Provide schedule to IDT. Schedule admission, quarterly and annual assessments. Provide addendums to the schedule to the IDT as needed as related to COTs, discharge assessments and SCSA (Significant Change in Status Assessments). *
c. MDS sections as assigned facilitate with IDT members the completion of assessments. *
d. Ensure the transmission of assessments per regulation. *
2) Facilitate the completion and updating of resident Care Plans within MDS and the IDT to reflect the resident’s most current needs. *
a. Provide ongoing leadership for data accuracy, data submission and computer applications of Resident Assessment, communicating regularly with the IDT. *
3) Participate in quality improvement initiatives as directed.
4) Provide necessary documentation of all activities to meet regulations. *
a. Maintain validation reports in a transmittal notebook organized by month. *
5) Support various other Nursing responsibilities as needed.

JOB SPECIFICATIONS
Education, Experience and Credentials
· Licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.
· One or more years’ nursing experience in a long-term care setting required.
· One year of MDS and care plan development experience preferred.
· CPR Certification preferred.

· Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.
· Possess effective problem solving skills.
· Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.
PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB
Incumbent must be able to:
· Perform light work, lifting up to 20 pounds, with frequent lifting/carrying of objects weighing up to 10 pounds.
· Push and move wheelchairs with residents of all weights and sizes for short and long distances.
· Stand, walk, twist, turn, bend, stoop, kneel, and reach frequently in the performance of resident assistance and nursing duties.
· Effectively communicate in the English language, including following oral and written instructions, in order to communicate with people inside and outside the organization.
· Work at a desk for extended periods and perform hours of work at at a computer.
· Apply manual dexterity and computer keyboarding and office equipment use.
· Perform repetitive movements of the upper extremities.
· Perform frequent use of hands for firm gasping and fine manipulating.
TOOLS AND EQUIPMENT USED
· Use standard nursing equipment such as thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.
· Use telephone, computer and standard office equipment.
WORK ENVIRONMENT
The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
