Job Description:The Stroke RN Navigator is a Registered Nurse who supports the Primary/Comprehensive Stroke programs cross-campus. This position provides targeted risk assessments and focused education to patients, families, and the community to maintain the Joint Commission Disease Specific Care (DSC) Stroke center certification and Center for Medicare and Medicaid Core Measures. In addition, the Stroke RN Navigator coordinates care across the continuum of care by collaborating with physicians and staff to assure optimal care is provided to patients with Transient Ischemic Attack (TIA), Acute Ischemic Stroke (AIS), Intracerebral Hemorrhage (ICH), and Subarachnoid Hemorrhage (SAH).

Education:

Graduate of an Accredited School of Nursing - Required

Experience:

2+ years Experience in Nursing - Neuroscience or related field in Nursing - Critical Care or

Nursing - Progressive Care or

Nursing - Telemetry - Required

Certifications/Licensures:

RN Registered Nursing - California Board of Nursing - Required

NIHSS National Institute of Health Stroke Scale - No Issuing Agency - Required

California Driver's License - Proof of Good Driving Record and current Car Insurance - Required

SCRN Stroke Certified Registered Nurse - American Board of Neuroscience Nursing - Required or achieved within six months of hire and/or

Certified Neuroscience RN - American Board of Neuroscience Nursing - Required or achieved within six months of hire

Modified Rankin Scale Certification - Required or achieved within six months of hire

Skills:

Demonstrated skill in operating a PC computer, skill using word processing, strong experience in Excel (e.g., pivot tables), database, e-mail, and Internet applications on a PC in a Windows environment.

Work Shift:10.0 - 07:00 - 18:00 No Waive (United States of America)

Pay Range:

$78.58 - $107.07Hourly

Offer amounts are based on demonstrated/relevant experience and/or licensure.Pay will be adjusted to the local market if hired outside of the Bay Area.
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word ‘exempt’ on it.
