RN Structural Heart Cardiothoracic Surgery Clinical Coordinator Boulder Community Health, Boulder, CO 80303 $37.89 - $56.83 an hour - Full-Time Days, 40 hours per week Boulder Community Health is seeking a RN Structural Heart Cardiothoracic Surgery Clinical Coordinator that is responsible to deliver and coordinate optimal patient and family experiences from diagnosis, through evaluation, treatment, and follow up for patients with coronary and structural heart disease and is recognized as a vital core to the quality and continuum of care offered through Boulder Heart. The Cardiovascular service line encompasses a multidisciplinary team from the specialties of Cardiology, Non-invasive and Invasive, Diagnostic Cardiology, and Cardiac Surgery who the Structural Heart/CTS Clinical Coordinator will act as a liaison between in facilitating the quality initiatives. Responsibilities •The Coordinator helps to drive a quality based, patient-focused program. A coordinator is devoted to managing the program implementation and execution, patient /physician notification components, education, and outreach. The Coordinator is the primary advocate of the program, both internally and publicly. •Collects and analyzes potential patient data from various channels, including echo alert data (inpatient & outpatient), cath lab data, hospital and ambulatory medical records. •Develops structured communication pathways to ensure identified patients’ data is communicated in a timely manner. •Offers assistance with referral coordination for patient who meet established criteria. •Provides follow up to each case referred to determine appropriateness of referral, timeliness of disposition, and customer/patient satisfaction, and takes immediate action to problem-solve whenever necessary. •Coordinates data collection for clinician decision making related to this patient population. •Maintains, manages and reviews data in valve tracking data base. •Provides patients with information about follow-up care with cardiology. •Assist in developing and revising educational materials for the referral base related to these disease processes. •Provides education to individuals within the care continuum of patients with cardiothoracic surgical needs and structural heart disease. •Assists in the development of annual QA plan and quarterly QA report. •Assists in the ongoing evaluation of service’s function and goals. •Updates database as patient progress through their disease process.
