The RN Transfer Nurse is responsible for coordination and communication of transfer requests for LCMC Health Adult Hospital Market (AHM) facilities by using problem-solving, critical thinking, and excellent customer service skills. This professional will act as a facilitator and decision-maker while applying nursing knowledge as a foundation for placement decisions and communications. This role will also be responsible for bed planning and throughput facilitation at designated LCMC Health Adult Hospitals.

Job Description

GENERAL DUTIES

Serve as the “point person” to facilitate incoming transfers/transports, maintain communication with all parties including but not limited to attending/accepting MD and referring facility.

Uses problem-solving skills to determine best course of action which will be satisfactory to the patient and referral health care team and optimize communication with the appropriate physician.

Utilizes nursing experience and education as foundations for gathering accurate clinical data to assure appropriate and prompt transfer and placement of the patient.

Responsible for documenting, tracking, and coordinating logistics for hospital transfers and bed planning.

Establishes and maintains a high degree of awareness and knowledge of hospital’s medical staff, services and programs.

Maintains professional and technical knowledge through continuous education and accepts new assignments and performs additional duties as requested.

EXPERIENCE QUALIFICATIONS

3 years experience in inpatient nursing / call center / critical care / ED


LICENSES AND CERTIFICATIONS

American Heart Association Basic Life Support/CPR

Registered Nurse License


SKILLS AND ABILITIES

Basic Nursing skills with high level critical thinking ability.

Exhibits excellent customer relationship skills including telephone and personal communication with all customer groups (physicians, administrators, nurses, case managers, and other team members).

Proficient in hospital computer systems for accurate data collection, monitoring and evaluation.
