Job Description

 

The Triage Registered Nurse is responsible for ensuring that patient care is coordinated and managed appropriately by directing hospice services/care via phone conference. This experienced nurse has a background with an emphasis on palliative care.

As scheduled, you will receive calls related to patient care and instructs patient/caregiver on appropriate interventions based on nursing knowledge. You will assess available options for interventions and uses critical thinking when choosing what intervention is best for the patient and/or caregiver. You will determine need for nursing visit based on EMR and telephone call and dispatch a visit RN/LPN appropriately You will facilitate the patient’s efforts toward self-sufficiency and optimal comfort care. This is a 100% Remote position. Typical schedule is Two(2) weeknights 5pm-8am, Every/other Friday 5pm-8am and Every/other Saturday and Sunday 8pm – 8am.
