The staffing and organizational plan consists of both site and outreach workers who work citywide to help unhoused and unstably housed people to be healthy and safe while homeless through case management, referrals to services, addressment of daily living needs, criminal justice navigation, and follow-up care coordination. Our staff will facilitate non-coercive service connection based on the client’s priorities; staff use motivational interviewing, unconditional positive regard, assertive engagement, trust and reciprocity to build relationships and meet client goals. The ROOTS Outreach Site Manager position will be responsible for the daily supervision of site staff and oversee the functioning of sites in order to support the unhoused and unstably housed individuals. Current ROOTS sites are located in the Northside, Downtown and East Liberty, respectively in Police Zones 1, 2, and 5. POSITION DUTIES + Supervises ROOTS Site Support Coordinators and other staff as necessary. + Provides constant support to assigned ROOTS site. Regularly collaborates with and supports all other sites. + Coordinates staff on site to ensure provision of services. + Establishes supportive relationships with people experiencing homelessness in order to assist them with housing needs, and co-occurring mental health, drug and alcohol, and physical health needs. + Manages active case load which includes supporting current and former client needs. + Documents all service encounters, client information, and outcome tracking in relevant software using proper case management protocols. + Tracks incident reports, emergency calls, and disturbances within the office. + Resolves technical needs and provides technical support. + Collaborates with internal and external stakeholders to provide education surrounding program objectives. + Facilitates connections with internal and external providers for people experiencing homelessness. + Ensures accurate data entry for supply distribution documentation. + Ensures coordination of follow-up care to facilitate secure service connections. + Attends weekly case conferencing with Pittsburgh Bureau of Police to coordinate care for clients. + Coordinates maintenance and facility operations needs of assigned ROOTs site. + Tracks office supplies and harm reduction supply inventory and facilitates ordering with appropriate staff. + Facilitates transfer of food donations. + Performs other duties as assigned or required.
