Hourly Pay Rate: $23.86 

Provides support to the Lead Regional Resource Development Specialist in the implementation of the waiver programs.  Participates in the daily operations of the programs and be able to reviews initial and revised service plans.  Inputs data into the Department of Health data base and tracks records, reports and information required by contract with the NYS Department of Health.  Also may be required to participate in completing assessments of participants and conducting training sessions for provider agencies.  Reviews all investigations of Serious Reportable Incidents and determines compliance with Quality Assurance standards.  Oversees and manages Housing Program on the program worked with the highest percentages. Write up and presentation at fair hearings if requested by director. Complete Health Commerce UAS training at least 15 level and manage case lists involving Headway of WNY.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

Independently completes assessments of participants and reviews service plans for completeness focusing on the needs and goals of the waiver participant. Makes decisions based on requirements of contracts (regulations) with the NYS Department of Health, and consistency with the needs of the participants. Review of Serious Reportable Incidents and compliance with Quality Assurance standards. Responds to calls from individuals with disabilities and seniors, family members, advocates, professionals and others requesting information regarding waiver services. Interviews prospective participants who indicate an interest in returning to the community.  Responsible for compiling intake information and details on participants. Tracks referrals and intakes and helps maintain an informational database vital to the analysis of the effectiveness of the waiver programs. Promotes the quality and availability of services sufficient to allow waiver participants the real opportunity of choosing providers. Manages Housing Program specific to Waiver Contact. Manage SRI reports Other duties as assigned.

 

MINIMUM QUALIFICATIONS:

Possess a baccalaureate degree in health or human services or related field. At least two years combined satisfactory professional experience in working with seniors or people with disabilities and in a managerial capacity.   May possess an Associate’s degree from an accredited college and five years of combined satisfactory experience working with individuals with disabilities and/or seniors and in a managerial capacity. Competence in Microsoft Office is mandatory. Must have a valid driver’s license.

 

SUPERVISORY RESPONSIBILITIES:

This position may be assigned supervisory responsibilities.
