Key Position Details:

This position involves close collaboration with infectious disease specialists from various health systems. The case manager will work under a Ryan White funding contract, providing services to HIV-positive individuals. The ideal candidate will have knowledge of HIV, experience working with HIV-positive individuals, and familiarity with community resources

Job duties include but are not limited to:

Client case load, documentation, statistical and data collection, resources (knowledge of and use of to assist clients with insurance/health care, housing, financial and basic needs).  Unit cost billing, safe sex education, community education, and travel.  Knowledge of computers, programs and the internet a plus. 

1.0 FTE (80 hours per two-week period)Hybrid position- will involve remote and onsite work at the North Memorial Infectious Disease Clinic and the Allina Health Uptown ClinicHours: 08:00 AM-4:30 PM, Monday through FridayFour-year college degree requiredHIV certification required; assistance to obtain certification will be provided if neededBenefit-eligible position

Job Description:

Provides ongoing case management and service coordination for clients by identifying needs and partnering with each to develop individualized care plans. Collaborates with interdisciplinary team regarding ongoing evaluation of client needs and status and adjust service plan and community supports as needed.

Principle Responsibilities

Provides ongoing case management, needs assessment and service coordination across a continuum of care, services and settings.Develops individual treatment plan and goals.Identifies desired individual services and health practices that enhance the care plan.Facilitates communication among team members, program and outside providers, referral sources, payers and community support.Completes documentation and assures that records are maintained.May facilitate completion of discharge planning.Provides ongoing evaluation of client needs and status and adjust service plan and community supports as needed.Identifies barriers and develops strategies to overcome barriers.Provides coaching and information to client and family.Advocates for client needs.Participates in quality/performance improvement activities related to case management and improved outcomes for clients.May participate in multidisciplinary care conferences.Other duties as assigned.


Required Qualifications

Associate's or Vocational degree in Social Work, Nursing, Psychology, Education or Vocational Rehabilitation


Preferred Qualifications

Bachelor's degree in Social Work, Nursing, Psychology, Education or Vocational RehabilitationExperience in health care, human services, or rehabilitation


Licenses/Certifications

Licensed Social Worker - MN Board of Social Work preferredCase Manager Certification preferred
