On an Average day you will:

Schedule:

Assign and coordinate patient visits for licensed and unlicensed staff, including providing necessary visit-specific details.

Schedule patient visits based on physician orders and various events such as Start of Care, Re-certification, Discharge, and more.

Manage visit-related changes, including cancellations, reassignments, and rescheduling.

Handle scheduling for events like Reauthorization and Intra-Agency Branch Transfers.

Complete tasks within designated timeframes and perform other duties as needed.

Administrative:

Verify and schedule OASIS collections for specific visit types and manage schedule deletions for inpatient transfers.Review coordination notes for follow-up actions and assist in reporting lab results to healthcare professionals.Participate in case conferences and document coordination of care.Monitor and resolve exception reports and unverified visits, ensuring timely synchronization with field staff.Process mileage change events and assist in QAPI (Quality Assurance and Performance Improvement) initiatives.

​Professionalism:

Align daily activities with Elara Caring’s vision, promoting its mission and policies.Ensure patient and staff confidentiality according to HIPAA.Maintain positive relationships with patients, caregivers, and healthcare providers.Participate in team meetings and ongoing training.
What you'll Need to Bring:

Education:

High school diploma or GED required; 2 years of college education in a business field preferred.

Experience:

Minimum of 1 year of practical experience; experience in patient scheduling within a home care agency setting preferred.

Skills & Knowledge:

Strong communication skills (written and verbal), with the ability to listen, convey information clearly, and respond appropriately.

Advanced time management and organizational skills to plan and prioritize tasks effectively.

Judgment capabilities for making informed decisions, knowing when to escalate issues.

Proficiency in Microsoft Office and ability to learn enterprise applications (e.g., Homecare Homebase, Workday).

Intermediate conflict resolution and customer service skills to manage patient expectations and address issues.

Basic problem-solving and financial management skills, including understanding the financial impact of scheduling.

Bilingual in Spanish/English preferred.
