**Employment Type:** Full time **Shift:** Day Shift **Description:** **Scheduler/CNA** **Location:** Norristown, PA **Status:** Full time **Shift:** Monday - Friday, 8 to 430, and weekends as needed **Incentive** 1st Day Benefits **Position Purpose:** This position is responsible for coordinating and scheduling external appointments for all participants at the PACE Organization (PO). The scheduler will prepare schedules according to the participant needs and the participant's care plan designed by the interdisciplinary team. Works closely with the transportation department, participants, caregivers, and center staff. This position is also responsible to complete home visits verify CNAs are following the care plan. Other duties will include providing training for new CNA’s and conducting supervisory home visits. May be required to work as a C.NA should the need arise. **Position Details:** Role includes both home care and facility work. **What you will do:** + Coordinates and schedules appointments for home care assignments for participants. + Works closely with the transportation department, participants, caregivers, and center staff regarding participant schedule. + Maintains complete and accurate participant schedules, including the amount, frequency, and duration of service that has been identified in the plan of care. + Notifies appropriate manager when requests for scheduling changes are received that require a physician's order. + Records all scheduling changes, updates, and deletions on the schedule daily. + Develop schedules for and conducts training for new CNA’s, which will include home visits. + Conduct home visits to oversee service delivery and monitor participant satisfaction and ensure CNA’s are following care plan. + May cover CNA shifts as needed for home care and/or facility assignments. **Minimum Qualifications:** + High School Diploma or equivalent required. Associate degree preferred. + Minimum of one (1) year scheduling experience required. Medical or patient scheduling strongly preferred. + CNA certification required. + 3 years of CNA experience required. + One (1) year of customer service experience serving as the first point of contact via email/phone/in person strongly preferred. + Minimum of one (1) year of documented experience working with a frail or elderly population or training onsite upon hiring. + Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.), experience with electronic mail and calendaring system. + Superior written/verbal communication skills. + Valid DL, insurance and use of personal vehicle required. + Excellent organizational, problem solving and customer service skills as well as ability to adapt to change and attention to detail. **Position Highlights and Benefits:** + Comprehensive benefit including 1st Day medical coverage, dental, vision, paid time off, 403B and educational assistance. + Access to daily pay and employee referral incentives. + Supportive environment with a patient-centered focus. + Opportunities for professional development. **Ministry/Facility Information** **Mercy LIFE Norristown** , part of Trinity Health PACE, provides high-quality care to seniors in the communities we serve.
