This position reports to the Scheduling Supervisor, Scheduling Support Specialist, or the Clinical Team Manager (CTM). The Scheduler performs clerical and scheduling activities that facilitate Brooks Rehabilitation Home Health operations. These activities include coordination of clinical employee schedules as directed by ordered clinical needs and patient direction. Managing scheduler-identified reports, as well as timely communication of patient changes to appropriate Home Health employees. Ensures timely initiation of care to keep assigned area in regulatory compliance. Scheduler should expect to work holidays and weekend coverage as needed. Complete other duties as needed when identified by department leaders.
Responsibilities
Ensures cases are filled within established time period Exhaust all means available to reach patient when contacting for scheduling communication. Assists with the referral process, as needed. Maintains schedule for services requested, and provided to patients. Responds to emergency calls and arranges schedule accordingly. Provides appropriate notification of schedule to employee, patient, contract agency, supervisor, and others as appropriate. Works in collaboration with Account Executives and Clinical Supervisors, and employees to meet patient needs. Completes required reports and documentation in a timely manner. Ensure safe and effective coverage of patient needs. Assigns appropriate employees to patients as directed by the licensed professional employee, care plan, and service request. Ensures employees receive complete and accurate directions and information about patient needs as directed by licensed professional employee, care plan, and service request. Reports any scheduling changes and/or patient concerns to supervisor. Notifies patient of changes and monitors employee performance in areas of dependability, responsiveness, timeliness, and patient concerns. Follows up with employees on scheduling problems encountered by the on-call employee. Notifies Supervisor and Clinical Team Managers (CTM) of clinical needs. Performs job in compliance with Brooks Rehabilitation policies and procedures, as well as community and professional standards. Verifies time records with schedules and missed visit reports. Respects confidentiality of information in the patient clinical record and only shares this information in accordance with Agency policy and HIPAA. Performs other related duties and responsibilities as deemed necessary.Qualifications
High school diploma or GED; advanced education desirable. Current employee experience and temporary Staffing Agency or in Home Health environment is preferred. Knowledge of medical terminology or a general medical background desirable. Demonstrates strong verbal, written, and interpersonal communication skills. Working knowledge of Microsoft Office products including Word, Excel, Outlook, and PowerPoint. Demonstrates time management, organizational skills, and ability to function with minimal direction.Scheduled: Full Time (MONDAY-FRIDAY)
