The Primary Purpose of Position
The school-based health center community health worker (SBHC CHW) is often the first person with whom a student or parent will interact, and assists in creating strength based, equitable partnerships with children and families to promote their resilience as well as coordinating daily activities and functions of the SBHC program. The SBHC CHW is the main administrative support employee for the SBHC and an essential member of the SBHC team.

Essential Functions
· Establish relationships with the student population, school staff, parents, and the community to increase awareness and promote School Based Health services
· Conduct community outreach to provide information/make referrals to/coordinate services, identify community resources that provide needed support for patients, provide health education to individuals and groups on prevention and public health risks and problems, educate staff/providers about target population and community characteristics and needs
· Screen students and families for health-related social needs (SDoH) and connect families to services and interventions when needed
· In collaboration with SBHC and school staff, establish and help maintain a Youth Advisory Council whose focus is on projects related to school health and wellness
· Responsible for the efficient day-to-day flow of SBHC patient visits. Including but not limited to the timely completion of patient registration, scheduling, check-in and out during scheduled visits, accommodating requests for walk-in appointments utilizing EHR as well as scanning appropriate documents into EHR
· Determine health insurance eligibility and help with health insurance enrollment and linking to a medical home
· Order and restock health center administrative supplies and tracking orders/sending packing slips for reimbursement
· Use appropriate technology, such as computers and scanners, for work-based communication according to North Shore Community Health requirements
· Maintain an organized tracking system for patient consents and filing system for up-to-date paperwork, screeners and patient handouts
· Prepare reports as requested by SBHC or NSCH staff
· Other duties as assigned.
· Works in collaboration and under the direction of the school-based site manager
Requirements
·Fluency in Spanish and/or Portuguese required
· Minimum of High school Diploma required
· Experience with Virtual Gateway, health insurance and enrollment, medical referrals preferred
· Possess knowledge of and experience in working with health-protective social infrastructure programs that provide income or material support
· Community Health Center and/or SBHC experience preferred
· Experience working with adolescents
· Excellent customer service, communication, time management and organizational skills
· Strong computer skills and ability to quickly adapt to using electronic health record, Microsoft Office applications as well as other internet platforms
· Demonstrates attention to detail, flexibility and ability to multi-task in a fast-paced environment
As a health center, NSCH ensures that all employees comply with the annual requirements for vaccinations and health screenings. All employees must be vaccinated against COVID-19 as a condition of their employment unless they have been granted a religious or medical exception.
