School-Based Locum Nurse Practitioner (.4 FTE)

Purpose 

The provider will perform independent clinical activities and collaborate with other clinicians and consultants when appropriate to achieve quality patient care and contract compliance. They will support population management and care coordination activities through a team-based approach. The provider will work in partnership and joint accountability with other team members in pursuit of Neighborcare Health’s mission, guiding principles and goals.

In this position you will:

Direct Clinical Work  

Provide patient-centered preventative care, as well as the diagnosis and treatment of acute and chronic conditions  Identify the need for referrals and provide overall care coordination in a team-based setting.   Prescribe, dispense, and administer medications as appropriate.  Role does not have support of a Medical Assistant, so medical provider will room patient, take vital signs, give vaccines/injections, administer medications, conduct Point of Care testing, perform phlebotomy, process lab samples, etc 

Indirect Clinical Work  

Assure accurate and timely completion of all clinical records  Completion of incoming paperwork and tasks for patient care (e.g., interpreting and acting on test results, coordination with internal teams, communication with outside specialists, review of prior authorizations or patient discharges)  Coordinate patient care with appropriate team members, including clinic staff as well as school staff, while following HIPPA and FERPA regulations. 

Key Skills, Knowledge, & Abilities  

Required for this job  

Ability to conduct all appointments independently without a Medical Assistant – this includes rooming patients, taking vital signs, doing Point of Care testing, administering vaccines and injections, administering and dispensing medications, phlebotomy and processing lab samples, cleaning instruments, etc.  Ability to work in a high pressure, time-sensitive, and complex health care environment  Ability to effectively work with individuals of varying ethnicities, socio-economic levels, varying languages, cultures and sexual orientations and gender identities, including experience in working with interpreters in a medical setting  Ability to provide confidential reproductive and mental health services according to the Washington State Confidentiality and consent regulations for adolescent health services. Ability to meet and comply with HIPAA/Confidentiality policies and procedures, including ability to conduct discussion of sensitive nature with staff and patients  Ability to build relationships in patient care while maintaining appropriate boundaries  Ability to communicate effectively in person, on the phone, or over email  Ability to develop collaborative working relationships with Neighborcare Health, school staff, and families in a way that foster a positive working environment with an emphasis on teamwork  Attention to detail and ability to organize, prioritize, and manage time and tasks independently  Ability to adapt to the needs of our patients and clinics  Proficiency in electronic health records (Neighborcare Health uses Epic) and proficiency in email/office applications (Neighborcare Health uses Microsoft Teams and Office 365).  Ability to work with supervision, receive instructions/feedback, coaching/counseling and or action/discipline  Ability to demonstrate predictable, reliable and timely attendance  Ability to consistently demonstrate trustworthiness and respectful behavior  Ability to apply the Neighborcare Health Customer Service Standards 

Required qualifications

Graduated from an accredited Program Licensed as a Nurse Practitioner, with Washington State Department of Health Board-Certified  Current BLS Certification  DEA License issued and active in the State of Washington  1 year of clinical practice experience in community health, FHQC, family practice (or 1 year of residency)

Preferred qualifications

Nexplanon Contraceptive Implant Certification   Bilingual skills   2 years of clinical practice experience in community health, FHQC, family practice, or school-based health

Compensation

 The wage range for this position is a minimum of $66.15/hour up to $98.04/hour maximum 

 Compensation will be determined based on years of relevant experience.
