Job Summary:

The LPN works under the general direction of the Clinical Nurse Manager (or their designee), in accordance with the objectives, policies, and procedures of the nursing department and established professional standards. Assists in planning and providing federal and state regulated care of surgical patients of all ages. Administer immunizations in accordance with policies/procedures/standards of care and legal limitations. Document in precise manner following established policy and procedures. Cross-train in policy and procedures of the Employee Health Dept. Occasionally travel within and between Oswego Health’s locations to perform various departmental duties.

Duties/Responsibilities:

Supervise the care of patients with direct accountability to RN charge nurse or supervisor. Collaborate with RN in development of nursing care plan; assist in planning, providing and evaluating patient care as a member of the health care team. Prepare and administers medication via all acceptable routes in accordance with policies/procedures/standards of care and legal limitation; administer specific medication, including appropriate documentation. Observe patients for reactions to drugs, treatments, signs and symptoms, or other unusual conditions and immediately reports to charge nurse. Immediately report any observed changes in the patient’s condition or behavior to the charge nurse or team leader. Record all patient care provided in a precise and detailed manner following established procedures for charting. Use computer programs, including Order Entry, per policy and procedure as assigned. Identify and appropriately use the chain of command to assist in resolution of unit, staff and patient concerns. Regularly check and maintain equipment and emergency crash cart per unit-specific policy. Maintain proper level of stock and emergency supplies as unit requires. Assist patients in ambulation; transports patients, demonstrating a concern for patient safety and utilizing proper techniques of body mechanics. Strictly follow principles of asepsis and infection control for routine cleaning and sterilization of equipment and supplies. Strictly follows principles of asepsis and infection control guidelines when performing patient care. Promptly answers patient call lights, whether that of assigned patient or not. Correctly identifies patient by name and ID band for all direct care, procedures or treatments. Routinely provides for patient safety through preventive measures (i.e. siderails, restraints as per physician order, low bed position and safe transport to other areas). Give and receive report per department-specific policy. Handle telephone information with courtesy, accuracy and respect for patient confidentiality; receives information and distributes messages appropriately. Communicate respectfully with patients and visitors; promptly respond to patient needs or requests for assistance. Maintain confidentiality in all patient, hospital, and physician-related information; demonstrate complete discretion when discussing patient information. Perform other duties as assigned.

Required Skills/Abilities (Unit Specific):         

Ability to cost effectively utilize resources and supplies. Ability to work effectively and communicate frequently with other members of the patient care team. Ability to communicate effectively, providing appropriate and accurate communication nonverbally, verbally and in writing. Excellent customer service attitude with effective listening skills. Ability to demonstrate positive, supportive and understanding behavior when in contact with guests (patients, visitors, clients, physicians, employees, volunteers, students, etc.)

Education and Experience:

Graduate of an approved LPN program. Current NYS licensure. Current certification in CPR. Willing to become certified for specialty duties such as NIOSH Approved Spirometry, respiratory fit testing, Occupational Hearing Conservation, and Certified Drug & Alcohol Technician. Must possess strong communication/computer skills.

Working Conditions:   

Requires unit specific orientation. Occasionally requires assignments to other units/shifts as hospital needs require. Requires frequent walking, standing, bending, lifting, pushing and pulling during the work day. Requires pushing beds, wheelchairs and recliners in excess of approximately 200 pounds with assistance. Requires lifting, carrying, repositioning, and transferring residents who may weigh in excess of 200 pounds using proper body mechanics. Requires assembling, using, and maintaining departmental equipment. Exposure to patient blood, urine, and other body fluid samples along with other hazardous materials Attendance at scheduled date and location ensures that job requirements are met. Actively participates in Quality Assurance, CQI, cross-training, in-service programs; attend all mandatory classes, meetings and committees as required; complete department-specific requirements/competencies; accept assignments to other units/shifts as hospital needs require. Complies with DOH requirements for annual health assessments and required immunizations.

Pay Range: $24.00-$26.40/hr

Employee salary is based on commensurate experience and other qualifications.
