Email: careers@communityhousing.org WHAT WILL I BE DOING? CHI is looking for a full-time Senior Case Manager for our Middle Country location. You will be responsible for all supervisory aspects of social work services within emergency housing setting. Under the direction of the Program Manager, the Senior Case Manager will work as part of an interdisciplinary team. The Senior Case Manager will supervise the overall running of the Case Managers for their site. The Senior Case Manager will ensure quality housing services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate. This is accomplished by performing the following functions: + Provide crisis intervention as appropriate. + Support direct case management duties as needed. + Provide regular supervision for Case Managers. + Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly). + Ensure that the program is providing quality services and is compliant with program requirements. + Promote sound social work practices through supervision including case consultations, case file reviews, and employee performance appraisals. + Ensure the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI (housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, admission and discharge sheets, census reports, etc.). + Inform Program Manager of deficiencies that require additional staff training. + Act as a liaison with agencies responsible for referring eligible clients. + Adhere to proper use of company resources (cell phones, office supplies, internet, etc.). + Act as a role model to Case Management staff in accordance with agency policies and procedures. + Provide staff with ongoing training and growth opportunities. + Participate in the hiring process, initial and annual employee evaluations, and disciplinary measures. + Responsible to oversee appropriate coverage and payroll procedures within department. Provide on-call coverage as needed. WHAT DO I NEED? + Education: BSW or MSW from an accredited university or related field of study required. + Experience: 2-5 years’ experience providing management services preferred. + Certificates or Licenses: Valid driver’s license. + Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus. + Computer Skills: Basic competency in MS Windows, MS Office, and internet usage. Knowledge of Foothold Awards System a plus. + Math Skills: Basic math skills required. + Physical Performance: Ability to tour properties, walk distances, climb stairs, etc. + Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls. + Other Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details. Must be even-tempered, have good negotiation skills, and the ability to meet unexpected deadlines. Effective time management skills. ANYTHING ELSE? + Salary: $62,400 annually.
