Senior Clinical Pharmacist for the Anticoagulation Clinic at Providence Milwaukie Hospital in Milwaukie, OR.
Position will also float to other locations within the Portland Area
Full-Time/Day Shift
The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required Qualifications:
Bachelor's Degree in Pharmacy from an accredited School of Pharmacy Or PharmD from an accredited School of Pharmacy Coursework/Training - Postgraduate Year One (PGY-1) Pharmacy Residency Or equivalent educ/experience Upon hire: Oregon Pharmacist License Upon hire: Pharmacy preceptor license in applicable state as applicable 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy ResidencyPreferred Qualifications:
