CHWs are front-line public health workers who are trusted members of and/ or have an unusually close understanding of the community they serve. This trusting relationship enables CHWs to serve as a liaison/ link/ intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Required qualifications:
Associate's Degree in Appropriate social service or health area. Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. Within 6 months (180 days) of hire Complete the Community Health Worker Training and be certified by the state of Oregon 1 year of Community Health Worker Experience or Experience working with non-profits and local communities.Preferred qualifications:
