In this role as a Senior Community Residential Counselor, you will: + Ensure quality services are provided to each person supported through person-centered planning and maximize opportunities for integration with the community. + Assist the Supervisor in monitoring and documenting the delivery of all services. + Implementation of person supported behavioral support plans/strategies and documentation of person supported behaviors. + Participate in the departmental on-call system including remaining on-call for the work week and participate in the weekend on-call system. + Assist in the development and monitoring of staffing plans to ensure each person’s health and safety, including the possibility of providing direct care when necessary. + And more!
