Responsibilities: 

Provide health related tasks under the supervision of a nurse in a way that is consistent with overall participant safety and accepted standards of participant care. When Assigned to the Wellness Center, Lead CSW will support the needs of participants by assisting with transportation to medical appointments. Majority of work assignments will take place at designated PACE center; however, alternate work locations may include assisting a PACE participant in the home, hospital or nursing home.The Lead CSW will provide clinical support to Clinic NP or covering PCP.

Salary: $20.8675 per hour

Education Requirements:

High School DiplomaCNA license in state of NY

Experience Requirements:

1 year of experience with geriatric population in a long-term care or home care setting6 months experience supervisory or leadership experience, preferredBilingual in SpanishMedical Assistant/Clinical background
