Specific Responsibilities:
This is a Senior Licensed Practical Nurse position assigned to the West Palm Beach and Northeast Health Center, a multi-program Health Access Center in an urban medically underserved population, managing patients with different medical problems. Work is performed under the supervision of the Senior Community Health Nursing Supervisor. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position has specific assignments in the Maternity, Family Planning, Gynecology, Latent Tuberculosis Infection (LTBI), Infectious Disease Clinic, Sexually Transmitted Infections (STI), Teen Clinic, Adult Immunization, Pediatric Immunizations, Refugee and other clinics.
This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures, Emergency Operations Command Center.Confidential Data Set(s):Clinical Information, Immunization Data, HIVe & TB, Social Security Data, Epidemiology, HIV/AIDS Case Reporting and Surveillance, School Medical Record, WIC, Employee Health Record, FL SHOTS, Electronic Health Record (EHR), Surveillance Tools and Reporting System (STARS).
Assists the registered nurse and/or physician in providing care and administering treatment to patient in all clinic areas: Maternity, Family Planning, Gynecology, Latent Tuberculosis Infection (LTBI), Infectious Disease Clinic (IDC), Sexually Transmitted Infections (STI), Teen Clinic, Adult Immunizations, Pediatric Immunizations, Refugee and other clinics, as assigned. Performs patient vital signs measurements including but not limited to, blood pressure, temperature, pulse respiration, BMI, height and weight. Assists the Provider in the collection of specimens for various laboratory tests. Documents vital signs in the EHR/medical record. Assists physician and nurse in examination room. Prepares examination room for patients by cleaning room after each patient. Performs hearing and vision screening using the titmus and audiometer equipment. Administers medication orally, subcutaneous or intramuscular as ordered by the physician and with the supervision of the physician or registered nurse. Assists the registered nurse in providing health information to patients/families and completing appropriate professional workers in assigned clinical area. Documents accurately according to professional nursing standard and the health department policy.
Serves as Medication Room Nurse at the Northeast Health Center and assist in the Medication Room at the West Palm Beach Health Center. Responsible for the maintenance of the State Pharmacy program including but not limited to Medication ordering, inventory control and disbursement to the patients. Orders and dispenses all medical supplies. Maintain sufficient inventory of medical supplies. Maintain up-to-date log items dispensed including surgical instruments. Washes and cleans all re-usable supplies including surgical instruments and sterilize upon return to supply room. Cleans autoclave machine as needed and monitor infection control. Accountable for all re-usable supplies not returned. Stocks all examination, nurse counseling, immunization and work-up rooms with appropriate supplies as requested and performs simple maintenance on equipment. Notifies supervisor of any problems in obtaining supplies ordered and of any faulty equipment. Under the supervision of a provider assist with case management and referrals.
Assist to maintain tickler files for needed report on PAP smears, mammograms and immunization results under the supervision of the Nursing Consultant. Assist with data collection and documentation as required for reporting purposes.
Performs EKGs as requested by the provider. Reviews, prints and attaches individual client appointment to the medical record as needed.
Assist in scheduling and rescheduling broken appointments.
Responsible for HIV pre and post testing counseling. Assists in the orientation and practicum planning for nursing students, medical students, residents and other groups. Participates in staff meetings, in-service programs and Quality Assurance program as assigned. Participate in outreach activities as needed. Perform other duties as assigned.
Required Knowledge, Skills, and Abilities:
+ Intermediate to Advance computer skills (Microsoft Word, Excel, Internet)
+ Critical thinking and problem-solving skills
+ Knowledge of physical assessment skills, application teaching and learning principles
+ Knowledge of nursing and public health principles practices and techniques
+ Knowledge and ability to proper wash, clean and sterilize surgical instruments
+ Knowledge and ability to collect routine laboratory specimen as directed by physician
+ Knowledge of Health Management System-Clinical Information
+ Knowledge of Electronic Health Record
+ Knowledge and ability to use routine/standard medical equipment, EKG machine, Titmus, nebulizer, pulse oximeter, spirometer and audiometer equipment.
+ Ability to communicate with and establish/maintain an effective working relationship with others, customer service skills, communicate verbally in a courteous and effective manner
+ Ability to plan, organize and coordinate work assignments for self
+ Ability to administer medication orally, subcutaneous or intramuscular
+ Ability to perform EKGs, hearing and vision screening
+ Ability to order and dispense medical supplies.

Licensure/registration/certification requirement:
+ This occupation requires licensure as a practical nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27.

+ Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
