Pay Rate: $20-$22.71
Shift: Monday-Friday 8:30am-4:00pm *temporary position*
The case manager assists seniors with gaining access to necessary services and supports appropriate to live independently in Senior Living Apartments. The Case Manager provides referral/linkage to and monitors appropriate programs and services for the seniors.
ESSENTIAL FUNCTIONS/RESPONSIBLITIES:
• Provides linkage and referral to services for tenants and families.
• Completes documentation of all tenant related services provided.
• Provides monthly site visits to senior sites to provide on-going case management services.
• Participates in case conferences and specific training programs/services with public and private agencies. Obtains at least 15 hours of approved in-service training annually
• Provides information and assistance to tenants and participates in tenant move outs as needed.
• Develops resident’s service plan if needed and reviews periodically.
• Maintains an up to date knowledge of senior tenant issues including, Medicaid, Medicare, HUD changes/revisions regarding Case Management and other appropriate community programs.
• Supervises students, volunteers, aides or other individuals as assigned
• Maintains confidentiality and process, maintains and utilizes potentially sensitive information in accordance with agency and departmental policies and
• Provide representation for benefit eligibility interviews – ie. Medicaid, Food Stamps, Medicare, Social Security etc.
• Provide representation to community organizations on behalf of the agency
• Provide Information and Referral to community members as needed.
• Advocate on behalf of tenant and family.
• Independently makes decisions concerning services and supports to for seniors considering independent living
• Educate tenants, families, and staff on available Community Resources.
• Acts as a liaison between community agencies, tenants and families
• Provides on-going monitoring of service delivery which includes routine telephone consultation of tenants.
• Provides crisis management in services as needed.
• Compile and maintains Resource Center.
• Complies with all agency policies and procedures.
• Other duties as assigned.
MINIMUM QUALIFICATIONS:
• Bachelor’s degree in social work, psychology or human services, BSW from an accredited institution preferred
• Two years of social work/services experience or equivalent service coordination and experience with seniors.
• Working knowledge of aging process and awareness of community services and programs
• Ability to organize, plan and manage caseloads
• Must have sound judgment, flexibility, dependability and good writing skills
• NYS Drivers License that meets agency policy
• Ability to handle multiple tasks simultaneously
SUPERVISORY RESPONSIBLILITES:
• Responsible for oversight of students, volunteers, aides or other individuals as assigned.
