Summary The Senior Living Director is responsible for creating an exceptional LHC Healthcare Service customer experience by providing support to clinical resources, therapists, and other associates of the network. Ensure delivery of quality care with timely scheduling and delivery with prompt follow up of care for rehab needs. Maintains the objectives, goals, and philosophy of healthcare delivery in conjunction with the LHC Group mission. Partners with agency clinical leadership to monitor efficiency levels within operations assuring prudent financial resource utilization. Ensures efficient and effective communication among community leadership and interdisciplinary LHC partners. The Position of Senior Living Director for LHC Group requires expertise in clinical practice, operations management, program innovation, growth strategies, employee recruitment and retention, understanding of company key metrics, leadership qualities, communication skills, and experience with conflict resolution. Position also requires assisting with, establishing, and implementing company policies that will allow LHC Group to provide outpatient services that integrate with other services lines, thus creating a synergistic model allowing for patients to receive rehab services through the continuum of care. Responsibilities Supports clinical resources and the Healthcare team to ensure consistent service delivery of LHC Healthcare programs at the highest quality for the patient and family. Assists in identifying clinical program needs and supporting implementation with community leadership and Clinical resource team. Meets consistently with Clinical Teams for the purpose of monitoring customer experience, quality of care delivery, and facilitating program growth. Works closely with Clinical Management and provides rehab expertise on delivering quality care for patient and family. Maintains involvement in the interview process for field level clinicians. Supports the on-boarding process of Senior Living rehab associates as well as Senior Living Management. Serves as an educational resource to the community regarding healthcare services. Coordinates available resources for staff and families. Markets LHC Group Healthcare Services to any appropriate audience. Participates in the continuous quality improvement program as required. Ensures regulatory compliance of any the certified Rehab Agency sites/Private Practice. Monitors and directs effective utilization of LHC Healthcare Services documentation standards and expectations. Analyzes Key Performance Indicators and problem solves to ensure quality patient care and outcomes. Reports on trends and activities using defined reporting processes and assists with action plans to correct, improve, or advance the identified trends and activities. Works in conjunction with local Senior Living Communities and Home Health agencies to ensure appropriate utilization, regulatory compliance, and clinical outcomes for patients. Maintains current knowledge of legislative, community, and payment regulations that affect agency goals and performance. Adheres to established confidentiality standards and ensures standards are maintained by all associates. Ensures compliance with all Infection Control, Universal Precautions, and OSHA standards for healthcare professionals. Responsible for compliance for state survey and other regulatory practices. Ensures agencies maintain a safe and clean work area and adhere to community and corporate safety standards. Participates in weekly/monthly Operations calls to review current business performance. Responsible for promoting an environment of collaboration on the team as well as across product lines. Education and Experience Must be a currently licensed Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Certified Occupational Therapist Assistant, Speech Language Pathologist, or Registered Nurse in at least one state that they are responsible for overseeing. Minimum of five years of experience, two years of which must have been in direct therapy care, multi-discipline therapy management experience preferred. Must possess an understanding of clinical operations, staffing, business development, program development, human resource functions, therapy contracts, and financial reports. Must have strong analytical, problem solving, organizational, oral and written communication, project management, and leadership skills. Must be proficient in the use of current electronic medical record and business technology software systems.
