Salary Range:$60,000-62,000
Position Overview:
Senior Peer Navigator will join Project Renewal’s outpatient addiction medicine and drug user health team. This role has been created to primarily address the care needs of persons at high risk of overdose morbidity and mortality, with funding from the CDC OD2A Local grant. Project Renewal provides low-barrier health care services for people who use drugs and/or struggle with addiction. This care centers patient individual health goals and includes connecting patients to medication for opioid use disorder, safer use strategies, residential treatment, HIV/Hep C treatment and specialty medical care such as pain management. Patient-centered care supports goals that range from managed or safer use to complete abstinence. The Senior Peer Navigator will report directly to the Nurse Care Manager of Drug User Health, and will work closely with the Vice President of Occupational Therapy (for oversight of grant metrics and reporting), Medical Director of Drug User Health and Addiction Medicine (for clinical oversight), and the Senior Director of Substance Use Services (for Residential Program oversight and support with interdisciplinary navigation).
The Peer Navigator will support patients at high risk of overdose and other adverse health events, outreach patients, collaborate on patient goals, and actively connect patients with appropriate health and harm reduction services. To achieve these goals, the Senior Peer Navigator will work closely with interdisciplinary teams, including occupational therapists, counselors, nurses, peers, medical/psychiatric providers, and residential aides/security personnel. The Senior Peer Navigator will be directly responsible for working within the OD2A and 820 Residential programs, but will also interface with multiple other programs at Project Renewal in the service of patient care.
Essential Duties & Responsibilities:
The essential duties of the Senior Peer Navigator include but are not limited to the following activities:
+ Provides peer support, counseling, and coaching
+ Collaborates with patient and medical team to identify care and treatment goals
+ Provides harm reduction education including safer use and overdose prevention
+ Provides harm reduction and safer use supplies to patients
+ Refers patients to harm reduction and substance use services
+ Escorts patients to harm reduction and substance use services as well as primary care, mental health care and specialty care
+ Supports retention in care, including community outreach, appointment reminders, check-ins, etc.
+ Documents services and support provided in electronic medical record
+ Communicates with medical team including providers (MD, NP, PA) and Nurse Care Manager via phone, email, and in case conferences
+ Coordinates patient engagement in multiple healthcare services and systems proactively and responsively, including by initiating contact between healthcare teams with patient consent
+ Supports Nurse Care Manager in organizing and updating a panel of high-risk patients
+ Outreaches patients who have been lost to follow up in an effort to reengage them in care
+ Supports patients in preparation for discharge from the PRI 820 Residential Program and after discharge to maintain their connection to care and services
+ Supports patients’ connection to care in transitions between institutions and modalities of care, e.g., after transfer between shelter sites, discharge from inpatient hospitalization, following release from incarceration, and other high-risk transitions.
+ Connects with agency staff at multiple levels and disciplines to improve overdose prevention and response efforts
+ Must be willing and able to navigate public transportation system to travel to shelter sites and community to outreach patients
Qualifications:
+ High School Diploma or HSE
+ Minimum of 3 years working with people who use drugs and experiencing homelessness
+ 2+ year experience as professional peer required
+ CRPA preferred
+ Maintains respect and compassion for all patients regardless of their active drug use or treatment compliance
+ Knowledge and passion for harm reduction and supporting people who use drugs
+ Interest in medication for opioid use disorder treatment and education as well HIV/HCV prevention and treatment a plus.
+ Spanish language is a plus
All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
